A Passport for Transition

You all know I love the idea of grab sheet passport for hospital and communication well...This is a chatter pack resource for children that could be used in either transitioning into a new setting /year change or you could adapt it to make a transition passport to take back into school after the lockdown break.. They come with some guidance and suggestions, good opportunity to get the kids involved in making one. Sue
